Adult Day Health Care Program

About the Program

If you know an elderly or disabled veteran who is ready for discharge from the hospital or nursing home but is in need of continuing medical and/or rehabilitative services; or a veteran who is living at home but who requires additional health and social services in order to avoid premature nursing home placement, then our Adult Day Healthcare Program may be the answer!

  • part of long term care program designed to provide patients with the level of care most appropriate to their current needs
  • therapeutically orientated outpatient day program
  • health maintenance and rehabilitative services to frail elderly or disabled veterans and veterans with dementia and Alzheimer's Disease
  • individualized care by health professionals and support staff
  • emphasis on helping participants and caregivers develop the knowledge and skills necessary to manage care requirements in the home
  • goal is to maintain or improve the health and functional status of participants
  • assist participants to remain in the community
  • enable families and other caregivers to continue home care for an impaired family member
  • care provided by an interdisciplinary team and intended to be used as the primary care medical delivery site for veterans participating in the program
